In-Home Assessments – Pathways for Aging performs comprehensive in-home assessments to provide an objective assessment of the home environment and functional status of your loved ones or clients. .Assessments and all services are provided in the home of the older adult whether they live in a private residence, senior apartments or skilled nursing facility.
Caregiver Coaching – From our many years of working with thousands of older adults, we have gained an understanding of the emotions behind their changing behaviors. We can help you better understand your loved one, so you can be a more effective caregiver. We do this in a process proven to be effective:
- Coaching – on-going one-on-one advice and counsel
- Support – we are here when you need us through email communication.
- Phone Consultations – we can schedule regular consults by phone, which can be helpful when more convenient. Call us at 314-395-7560 for an appointment.
Care Management – If you feel you and your loved one could benefit from more care and support,we provide services for a range of different needs as well as recommendations, assistance, monitoring and reporting for a loved one. All services are provided in the home of the older adult whether they live in a private residence, senior apartments or skilled nursing facility. You can determine how often you need us, controlling the cost.
On-going Care Management can be provided intensively for a short-term issue or regularly on a weekly, bi-weekly, or monthly basis. The number of hours and services provided are tailored to meet each client or family’s specific needs. We also have multi-hour packages for cost savings.
Family Coaching & Mediation – Because we all have different personalities, perspectives and relationships; an objective professional can help guide the discussion toward what is best for a relative. We know how to facilitate a meeting with diverse opinions — even strong emotional opinions! We can help your family, too.
