Pathways for Aging was founded to help older adults age positively. That means we do all we can to help make the aging process easier — for older adults, their families, and their professional caregivers.
Pathways for Aging serves older and disabled adults currently living in the St. Louis, Missouri Metropolitan Area. Our service area includes St. Louis City and County, St. Charles County, and Northern Jefferson County. We also have a Geriatric Care Manager serving Southwest Illinois. We also work with family members who live out of state if their relative currently resides in the St. Louis area or needs assistance moving to the metropolitan area.
Our Services Include:
A comprehensive In-Home Assessment can provide you with an objective opinion about the current home situation and provide you with the information you need to make an informed about decision about whether your loved one can be supported to live safely in their home or needs to transition to senior housing. We offer short-term and long-term recommendations and can assist you in planning accordingly.
Care Management means we assess, recommend and provide any help needed by the older family member. We can assist and support your aging loved one in remaining in their own home, oversee and advocate for their care in senior housing, and collaborate with all involved and facilitate a team approach to caregiving.
Caregiver Coaching for family members, neighbors, friends who need a little help, support, guidance or resources in determining the best course of care for the older adults in their lives, while also caring for themselves.
Sometimes families just need an objective, professional facilitator to bring various opinions together to one best course of action. We help family members move toward a positive experience for their aging parent.
Psychotherapy is provided by experienced, licensed Clinical Social Workers specializing in the care of adults struggling with the changes and losses they may experience as they age. Our therapists also offer counseling for caregivers burdened with stress or depression.
Our professional expertise and experience in the mental health care of senior adults validates us as an invaluable resource for other professionals serving this community including Physicians, Attorneys, Financial Advisors, Trust Officers, Clergy and Professional Caregivers.
Pathways for Aging provides educational presentations for professionals, caregivers and community groups on various topics relating to aging and caring for aging adults.
