Prior to the Family Meeting, we may recommend that we conduct a Professional Geriatric Evaluation of your parent/family member(s) in the comfort and security of their own home. With this understanding, we are better equipped to be objective in listening to various family members and helping you determine your BEST course of action.
The Family Meeting – We can host your family meeting in our office, or help you find a convenient and unbiased location. It is best if all family members are present. Those who are out of town can be dialed in via phone or video conference. Depending on the parent’s situation, they may or may not be present. To assure a fair and balanced discussion, all family members have the opportunity to voice their own opinions and concerns as well as assuring the older adult’s voice and perspective is considered. Our experience in facilitating group discussions for the purpose of an agreed defined outcome will shine through.
After the Family Meeting – We can prepare an objective, professionally written Care Plan Report outlining Recommendations, Short-term and Long-term Goals, Action Steps and Resources as agreed to in the Family Meeting. This may include a list of recommended home care services or living facilities for your parent.
